If your organization is interested in joining the Care Transitions Network, review and complete our Enrollment Agreement. Return it to CareTransitions@TheNationalCouncil.org for full execution.
- A team member will return your fully executed Enrollment Agreement, plus a template to record your Eligible Professional information and a Practice Assessment Tool. The Eligible Professional list and Practice Assessment Tool must be completed and returned within 30 days.
- Following receipt of both documents, your practice will receive a payment equal to $200 for each unique Eligible Professional who enrolled in the Network. As your practice progresses through the five phases of transformation, it will receive additional payments. Each enrolled practice is eligible to earn up to $1000 for each unique Eligible Professional enrolled in the network.
- A member of the Care Transitions Network will facilitate an onboarding call to review your Practice Assessment Tool, discuss your practice’s priorities for quality improvement, and ensure that your practice has the technical assistance and resources it needs to progress through the phases of transformation.

The Care Transitions Network is made possible by a grant made to the National Council for Behavioral Health by the Centers for Medicare and Medicaid Services, in a partnership between the National Council, Montefiore Medical Center, Northwell Health (formerly North Shore-LIJ Health System), the New York State Office of Mental Health and Netsmart Technologies.
