Healthcare Communities

When you enroll in the Care Transitions Network, you will receive national information and content from our information portal – Healthcare Communities. HealthcareCommunities.org is a feature-rich knowledge management system and community portal that hosts open communities in support of healthcare transformation. The listserv via the Healthcare Communities portal will connect and allow all enrolled organizations and providers to share suggestions and questions with each other.

Joining the CareTransitions Listserv

As an enrolled organization, you will need to complete two final steps to join the CareTransitions Listserv via the Healthcare Communities Portal. Your organization and provider information has already been uploaded to the Healthcare Communities portal. You will receive an email from the Healthcare Communities portal to finalize your enrollment by:

  1. Creating a username
  2. Creating a password

Inviting staff to the CareTransitions Listserv

The Care Transitions Network encourages all enrolled providers to invite their staff to the CareTransitions listserv via the Healthcare Communities portal to receive relevant announcements and to share questions and ideas. For interested staff, please follow these steps to join our listserv.
