CoE Office Hour: Introducing New Tools to Advance Your Integrated Care; UIC’s Solutions Suite for Health & Recovery

Promoting the integration of physical health, mental health, and substance use recovery services is critical for improving health outcomes for individuals living with co-occurring health conditions. To address a growing need for integrated health care and related innovative tools, the University of Illinois at Chicago (UIC) Center on Integrated Care, has developed several free tools in a ‘Solutions Suite’ that practitioners can use with clients, to foster health improvement, wellness and recovery.
