Strengthening Integrated Care Systems and Cross-agency Collaboration

As health care systems work to address the increasingly complex needs of individuals with cooccurring physical and behavioral health conditions, integrated care models have become essential to improving service access, coordination and health outcomes. Join us on Wednesday, May 28, 3-4 p.m. ET, for a discussion about the role of robust cross-agency collaboration in delivering strong, sustainable integrated care.

Strengthening Integrated Care Systems and Cross-agency Collaboration, from the Center of Excellence for Integrated Health Solutions, will explore how integrated care can improve access, streamline service delivery and support better health outcomes — while addressing common workforce challenges such as limited pipelines of trained professionals, salary disparities, credentialing barriers and burnout.

In this webinar, we’ll discuss the development and sustainability of integrated care systems, with a spotlight on real-world implementation strategies and workforce solutions. The session will spotlight Michigan’s Substance Use Disorder Health Home (SUDHH) as a leading example of an integrated care initiative that brings together behavioral health, primary care and recovery support services.

Leaders from Michigan will share their journey in building the SUDHH program — from assessing community needs and establishing cross-sector partnerships to securing funding and addressing early workforce challenges. Their presentation will highlight how they’ve designed a sustainable, person-centered model that supports long-term recovery and whole-person care.
