Rachelle Glavin has served as the Director of Clinical Operations at the Missouri Coalition for Community Behavioral Healthcare since 2008. Ms. Glavin began her work at the Coalition as the Disease Management Coordinator for the DMH Net Disease Management Initiative, one of the first integrated care initiatives between the state Medicaid authority, Department of Mental Health and the Coalition. She also assisted in the development and implementation of the metabolic syndrome screening policy and Disease Management 3700 Project, which received the 2012 Governor’s Pinnacle Award for Quality and Productivity. Ms. Glavin continues to support the development of healthcare homes and integrated care in the community behavioral health setting in several areas, including program policy and training initiatives.
