Population Health Management Strategies

Join us for a two-part webinar series that will explore improving health outcomes for both individuals and populations, first by exploring care coordination practices, and then by exploring population health management strategies.

The second webinar in this series will focus on Population Health Management, which considers all clients in a population, from low-risk, healthy individuals to high-risk individuals with chronic conditions. This approach requires the capacity to utilize data to pursue a wide range of activities to systematically track and manage health conditions, treatment access and use, and effectiveness of treatment and services to understand population health priorities. This session will explore how to leverage common EMR-based mental health and substance use screening tools, level of care determination tools, and risk stratification tools to better inform behavioral health and primary care integration, with a focus on identifying and addressing healthcare disparities.
