Building System-wide Support for the Collaborative Care Model

  • Secure behavioral health integration buy-in at varying levels within a health system when transitioning to CoCM.
  • Recognize infrastructure and workflow considerations that support the long-term sustainability of behavioral health integration and CoCM in large health systems.
  • Apply lessons learned from the CoCM rollout in a large health system to your implementation planning and troubleshooting.
