Delivering Together: Integrated Approaches to Perinatal Care Across Health Care Organizations

Perinatal mental health and substance use challenges have contributed to the maternal health crisis in the United States over the past two decades. Join us Sept. 22, 3-4:30 p.m. ET for a closer look at a robust, person-centered care coordination model that transformed a rural health clinic’s perinatal care delivery.

Care providers from the Northern Montana Provider Office Practice and Rocky Boy Health Center will tell how they work together across vast rural regions to improve maternal, infant and family outcomes through behavioral health integration and their cross-system partnership. You’ll also learn to recognize opportunities and address challenges with coordinating mental health and substance use care during the perinatal period.

This session is intended for obstetric providers, family medicine providers, care managers, care coordinators, mental health providers, substance use recovery providers, public health leaders, researchers, health managers and insurers.
