As more communities embrace the value of integrated care, providers are navigating implementation challenges, especially when building shared models across distinct organizations. In this webinar, leaders from Colonial Behavioral Health, a full-service community services board, and Southeastern Virgina Health System, a Federally Qualified Health Center (FQHC), will share how they designed a co-located model for integrated primary care and behavioral health.  

Integrating care offers powerful opportunities to improve outcomes, but the complexity of planning can be a barrier to implementation. From adapting established models to new settings, to navigating multiple organizational customs and workflows, this session’s presenters will take a candid look at their planning process and lessons learned along the way. 

This session will highlight how a shared vision, strong partnerships and thoughtful design can lay the groundwork for successful integration and offer insights for teams at any stage of the process. Participants will leave able to: 

  • Describe key planning steps taken to build a new, co-located integrated care facility. 
  • Recognize how existing models, such as Cherokee Health Systems, can be adapted to achieve successful and sustainable cross-organizational integration. 
  • Identify strategies for successful cross-organizational collaboration in integrated care planning. 

This session is ideal for community mental health centers, FQHCs, integrated care teams and system leaders exploring new or expanded models of integration.
