Zufall Health

Zufall Health Center

 

Zufall Health is a 501(c)(3) Federally Qualified Health Center (FQHC) and trusted safety-net provider of high-quality affordable medical and dental care to more than 30,000 of the most vulnerable, underserved residents of northwestern and central New Jersey. Over the past two years, with funding support from Johnson & Johnson, Zufall has developed a viable model for behavioral health – primary care integration.

Engaging more than 300 patients with serious mental illness (SMI) as primary medical care patients annually, Zufall has overcome critical access barriers such as lack of transportation in a rural service territory by using mobile medical and dental vans to serve isolated patients with SMI. Zufall’s integrated care model has overcome several programmatic challenges and has persevered by continuing to innovate, implement new approaches and refine integration processes.

Since 2009, Zufall has been in partnership with Saint Clare’s Medical Center – the largest provider of behavioral health services in the region. In 2016, Zufall provided primary care/behavioral health navigation to 389 patients with SMI for a total of 2,193 visits.

Zufall’s patients are among the most vulnerable residents of the region.

  • 79 percent have incomes below 100 percent of the Federal Poverty Level (FPL)
  • 96 percent with have incomes below 200 percent of the FPL
  • 51 percent are Medicaid beneficiaries
  • 27 percent are covered by Medicare
  • 15 percent have no third-party coverage
  • 6 percent are privately insured

Approximately half of the patients are black, more than one race or Hispanic and 30 percent require services in another language. All Zufall’s patients are diagnosed as seriously mentally ill.
