Elements to Integrated Care Success
Several key factors contribute to the success of Zufall’s program, including leadership commitment to incorporating behavioral health into primary care services, identifying opportunities for grant funding, devoting staff and resources to integrated care and developing and maintaining supportive relationships between health systems and community organizations.
Leadership and management staff at Zufall have a global vision of the services provided with integration of primary care and preventive services in all their programs, including behavioral health services, dental services, enabling services, clinical pharmacy, 340B medications, etc.
A key component to the success of Zufall’s integrated care is a dedicated patient navigator who serves as a liaison between the primary care providers and the behavioral health clinical staff. The patient navigator serves as a trusted advocate for behavioral health patients, established through years of integration and collaboration with the behavioral health team at the Saint Clare’s site.
The primary care team is empowered and receives training on behavioral health issues and conditions, including a Grand Rounds where the director of behavioral health services at Saint Clare’s speaks on the role of the primary care provider in treating common behavioral health conditions.
The quality assurance/performance improvement (QAPI) program involves behavioral health in all its improvement processes and provides dedicated time and resources for participation in collaboratives and initiatives.
