Care Coordination and Referral Partnerships in the Integrated Care Field

Join the Center of Excellence for Integrated Health Solutions on Tuesday, March 31, 3 – 4pm ET for an engaging webinar focused on strengthening care coordination and referral partnerships across the healthcare continuum. This session will also explore how health information technology (HIT) can be leveraged to streamline communication, support real-time information sharing, and reinforce continuity of care across physical, behavioral and social health settings.
