Introduction and Exploration of Collaborative Care
Join the CCBHC-E National Training and Technical Assistance Center on August 14, 12-3 p.m. ET, for a Collaborative Care Model (CoCM) Intensive. Collaborative care is an evidence-based approach designed to support people with mental health and substance use challenges in the primary care setting.
For CCBHCs, the model provides opportunities to fund psychiatric staff and registry use for population health management initiatives. It uses a treat-to-target approach, in which a behavioral health provider and a consulting psychiatrist work with the primary care provider.
This two-session series is for sites that are interested in learning more about the model, want to learn a proven 90-day implementation approach, or are looking to optimize their existing CoCM work. Participants should be CCBHCs that have primary care on site or are also community health centers (e.g., FQHCs, FQHC lookalikes).
