Applying New Peer Workforce in Integrated Care
Join the Center of Excellence for Integrated Health Solutions on Tuesday, May 26, 2–3 p.m. ET for a webinar introducing the Collaborative Care Model (CoCM) and its application in school-based health centers (SBHCs). This session will highlight how the CoCM’s team-based, measurement-guided approach strengthens the integration of behavioral health services into primary care. With a focus on pediatric and school-based settings, the session will cover practical considerations such as staffing models, workflow adaptations, and data systems that support integrated care.
Participants will explore real-world implementation examples from North Carolina and South Carolina, featuring case studies from the Medical University of South Carolina and Blue Ridge Health.
After this session, participants will be able to:
- Recognize common challenges to CoCM implementation in school-based settings and strategies to address them.
- Describe the core components of CoCM and how they support integrated behavioral health and primary care in SBHCs.
- Identify implementation strategies from Carolina-based case studies that support successful adoption of integrated care.
