Implementing the Psychiatric Collaborative Care Model (CoCM) requires more than clinical expertise—it demands intentional, structured change management across teams, workflows, and organizational culture. This 60-minute webinar on Tuesday, Sept. 1, 2026, 2-3 p.m. ET will provide a practical roadmap for leading change in primary care settings to successfully design, launch, and sustain Collaborative Care. Drawing on established change management principles, participants will explore how to create urgency, define value, and align CoCM with broader organizational priorities such as quality improvement, access, and cost containment.
The session will walk through key steps including building a strong coalition across roles (primary care providers, behavioral health care managers, psychiatric consultants, and leadership), developing a compelling vision, and communicating that vision effectively through data and storytelling.
Learning Objectives
By the end of this webinar, participants will be able to:
- Describe core challenges of practice transformation and explain why implementing Psychiatric Collaborative Care requires a structured change management approach.
- Develop strategies to create urgency and define value for CoCM by aligning the model with organizational priorities such as improved outcomes, patient access, and cost efficiency.
- Identify key stakeholders and build a strong implementation coalition, including securing buy-in from leadership, primary care providers, care managers, and psychiatric consultants.
- Articulate a clear and compelling vision for Collaborative Care that supports both clinical and operational goals.
- Apply effective communication strategies to promote adoption of CoCM, including crafting a concise “pitch,” using data to demonstrate impact, and leveraging patient and provider stories to build engagement.
- Recognize opportunities to align Collaborative Care with other initiatives (e.g., value-based care, quality metrics, population health) to strengthen sustainability and organizational support.
