Peer Impact Panel: Advancing the Role of Peers in Integrated Care Systems
Join the Center of Excellence for Integrated Health Solutions on Wednesday, May 27, 2-3:30 p.m. ET, for a discussion that will explore how people with lived experience are transforming integrated, whole-person care.
With real-world examples and system-level perspectives, the panelists — peer services leaders, health system leaders and researchers — will explore how peer support strengthens multidisciplinary teams within bidirectional integrated care. The discussion will cover peers’ impact on shared workflows, shared care, and coordinated support for chronic condition management and overall wellness.
Afterwards, participants will be able to:
- Define and describe bidirectional integrated care and the benefits of embedding peer support roles within multidisciplinary, integrated care teams.
- Examine the unique identity, values and contributions of peer professionals in integrated care settings.
- Identify system-level strategies and emerging practices that improve engagement and outcomes in integrated care.
