Join the Center of Excellence for Integrated Health Solutions and the Primary Care Development Corporation on Aug. 5, 1-2 p.m. ET, for a session exploring how integrated care can support treatment for individuals experiencing homelessness. Presenters from the Community Health Center of the New River Valley will share practical strategies, including approaches and on-site supports that reduce challenges and improve access to coordinated, whole-person care for experiencing homelessness.
After this webinar, participants will be able to:
- Explain how integrated care can support whole-person health, treatment and recovery for people experiencing homelessness.
- Identify integrated systems, practices and care processes that increase access to care for people experiencing homelessness.
- Describe how office-based addiction treatment (OBAT), wraparound support, peer support and community partnership can be aligned to support integrated care and recovery.
