Integrated Care in Rural Communities: Partnerships with Hospitals & Community Based Behavioral Health Providers

Join the Center of Excellence for Integrated Health Solutions and the American Hospital Association on Tuesday, July 21, 1–2:30 p.m. ET for a webinar highlighting a rural New Hampshire partnership between Weeks Medical Center and Northern Human Services, the region’s Community Mental Health Center. Together, these organizations are implementing an integrated care model to improve access to primary and behavioral health care in their community.

Participants will hear how this collaboration has put a shared vision for integrated care into practice through delivering early, coordinated access to services across the care continuum. Speakers will outline the steps taken to define roles, optimize care delivery and provide wraparound supports that strengthen patient outcomes.

By the end of the webinar, participants will be able to:

  • Understand how a hospital – Community Mental Health Center (CMHC) collaborative in rural New Hampshire was developed and identify steps to initiate a similar partnership in a rural community.
  • Explain the value of embedding behavioral health in primary care and apply assessment approaches to determine a patient’s need for CMHC services.
  • Describe strategies hospitals and CMHCs use to improve timely access, strengthen care continuity and support patient outcomes in rural settings.
