CCBHC Transformation

Certified Community Behavioral Health Clinic (CCBHC) Transformation (CCBHC-T) is a program to support state governments with scaling, strengthening and sustaining their CCBHC demonstrations. CCBHC-T provides vital and comprehensive technical assistance to states and clinics to advance the vision and impact of the CCBHC model.
The three-year initiative is led by the National Council for Mental Wellbeing in partnership with the National Association of State Mental Health Program Directors.

Three states — Illinois, Kansas and Michigan — will receive extensive support and state-based capacity building to help strengthen community-based care, expand the behavioral health workforce, and ensure more people get the help they need—when and where they need it. This effort will strengthen CCBHC services, invest in the behavioral health workforce and ensure sustainable, high-quality care for hundreds of thousands of people in need.
State Impact Reports
Through the CCBHC-T initiative, the National Council and NASMHPD are partnering with Illinois, Kansas and Michigan to support the growth and long-term sustainability of the CCBHC model. Funded by Ballmer Group, the initiative provides technical assistance designed to strengthen community-based behavioral health care, expand workforce capacity and improve access to services. CCBHC Value Proposition Toolkit
Communicating the impact of the CCBHC model is essential for building stakeholder awareness and support. This toolkit helps state teams develop clear, consistent messaging by identifying key audiences, organizing data and stories, crafting stakeholder-specific messages and planning communication activities. Designed for teams overseeing CCBHC design, development and implementation, the toolkit provides practical prompts and tools to help states tell the story of how CCBHCs are improving access to high-quality behavioral health care.
