Collaborative Care Model

Over the past few decades, integrated care has proven effective for people with co-occurring primary health and behavioral health needs. The Collaborative Care Model (CoCM) is the most widely studied integrated care model. Consistent evidence shows that it improves depression and anxiety outcomes across many populations and settings. It has been successfully adapted for groups such as children, trauma survivors, people with chronic conditions or substance use disorders, and those who need maternal mental health care. CoCM has been effective in real-world settings like community health centers and rural clinics. It is also cost-effective and offers strong potential for health care savings.

Center of Excellence for Integrated Health Solutions integrated care experts and training and technical assistance (TTA) partners at the Meadows Mental Health Policy Institute and American Psychiatric Association provide free TTA on CoCM. They draw on extensive experience supporting health systems, including those that serve rural communities, older adults, children and youth. We offer ECHOs, learning collaboratives, webinars, resource development and direct TTA consultations, with a special focus on CoCM implementation and sustainability.
