This CCBHC locator page is regularly updated with an interactive map and downloadable PDF list of Certified Community Behavioral Health Clinics by state. The goal of this page is to provide a valuable and accurate resource for finding Certified Community Behavioral Health Clinics wherever they exist in the USA. In fact, the CCBHC network has grown so fast that now nearly every state has one.

Established by the Excellence in Mental Health and Addiction Act, Certified Community Behavioral Health Clinics (CCBHC) are specially-designated clinics that provide a comprehensive range of mental health and substance use services. In October 2022, the CCBHC model was expanded nationwide, with planning grants available to help more states with implementation. These grants use funding authorized by the Bipartisan Safer Communities Act. CCBHCs serve anyone who walks through the door, regardless of their diagnosis and insurance status.

Today, more than 500 Certified Community Behavioral Health Clinics and CCBHC grantees are operating in 46 states, plus Washington DC and Puerto Rico. Please use the interactive map and list of current CCBHCs by state below to learn more about where and how CCBHCs are implemented nationwide.

CCBHC Locator Map

Use the interactive map below of the over 500 CCBHCs operating across the United States. These locations are either CCBHC-E grantees, clinics participating in their states’ Medicaid demonstration or part of independent CCBHC programs run by their state.
