Case to care training consists of five hours of live virtual training supplemented by four asynchronous webinars designed to increase health literacy. The content of the curriculum can be adapted to your organizational needs

  • Mental health and/or substance use disorder services 
  • Primary care 
  • Integrated behavioral health and primary care, including CCBHCs and health homes 

LEARNING OBJECTIVES

Case to Care Management training will expand your case managers’ capacity in health care navigation, build on their existing skills in health behavior change and help them gain a better understanding of the common health problems and basic interventions for individuals with serious mental illness and those with chronic health problems. Case managers will take a deep dive into:

  • Current health care trends impacting their role.
  • Differences between physical and behavioral health care cultures.
  • Strategies to build strong partnerships with primary care providers.
  • Tools to help people change their health behavior.

CURRICULUM

The robust Case to Care Management training curriculum will expand your case managers’ capacity in health care navigation, build on their existing skills in health behavior change, and help them gain a better understanding of the common health problems and basic interventions for individuals with serious mental illness or chronic health problems.

  • Current health care trends and national forces impacting their role and driving change. 
  • The role of case managers in supporting health care navigation. 
  • Differences between physical and behavioral health care cultures. 
  • Strategies for mental health and substance use providers to build strong partnerships with primary care providers. 
  • Tools to help people change their health behavior. 
  • Principles of team-based care and data-informed care. 
  • Conceptual framework for change: health homes, chronic care model. 
  • Rapid cycle change principles applications for health behavior change, goal planning and documentation.
